The first and most important step of the application process is discussing with your physician the use of medical cannabis in the treatment of your condition. This form must be completed and submitted to the Department of Health by your doctor. You must meet with your physician for your consultation within 90 days of submitting your certification form.
2. Complete Application
Completely fill in the application with requested information, choose medical cannabis dispensing organization, and complete with a signature. There are optional pages that request demographic and caregiver information, however those are not required for to complete the application process.
3. Application Fees
There is a non-refundable fee due at time of application processing. Fees are: $100 for a one year registry card, $200 for a two year registry card and $250 for a 3 year registry card.
* Reduced fee available for veterans or anyone enrolled in federal Social Security Disability Income (SSDI) or Supplemental Security Income (SSI) disability program. Reduced fees are: $50 for a one year registry card, $100 for a 2 year registry card and $125 for a 3 year registry card. Veterans, include a copy of your DD214. SSDI/SSI recipients, include a copy of your benefit verification letter, dated within the last year.
4. Send In Approved Photo
Submit a 2×2 inch passport-approved self photo. Be sure that you are by yourself, facing the camera head on, and your entire face is showing. No selfies!
5. Proof of Identity
Submit a copy of a valid, government issued photo ID that has not yet expired.
6. Proof Of Illinois Residency
Two items will successfully prove your state residency. Bank statements, utility bills, state ID, driver’s license and voter ID cards are all acceptable. The addresses on each of these documents must match the address of your application.
7. Fingerprint Form And Receipt
This form must be completed and signed, and also include your Transaction Control Number (TCN), along with the receipt from your fingerprinting. The application must be sent within 30 days of your fingerprinting.
Send in a copy of your DD214 and the reduced application fee. Fees are: $50 for a one year registry card, $100 for a 2 year registry card and $125 for a 3 year registry card. If you are receiving care at a Veterans Affairs (VA) facility, submit medical records from the VA about treatment for your qualifying debilitating medical condition from the past year, instead of a Physician’s Written Certification Form (step 1).
